Dear Parents,

This week the Department of Health will be reviewing the immunization status of all students here at Wagner Middle School. As a reminder, all students who have turned 11 need a Tdap shot, and all 7th and 8th grade students need a MenACWY shot in order to be in compliance.
If your child receives a letter indicating a missing vaccination, this week or next, please make a medical appointment to get the missing immunization ASAP. Many doctors' offices will let a child come in and see the nurse for an immunization, without a doctor's visit that day.
Please only send in immunization forms when asked. If you do NOT receive a letter, you can assume that we have your current form and it is up to date. 

Please make sure your student's OSIS (Student ID) number is printed clearly on the form.
Your student may bring in a paper copy, or you may email a PDF to Ms. Deane:
